Related Notices

This Privacy Notice should be read together with the Website Cookies Policy, the Website Terms of Use, and any notices displayed on individual website forms or features. Together these documents describe how the agency handles information collected through its public website. This Privacy Notice applies only to the public website; it does not replace a HIPAA Notice of Privacy Practices and does not describe the agency's complete practices for client records, employee records, or other confidential information maintained outside the public website.

1. Purpose of This Notice

This notice explains what information Seeding Hope Home Healthcare Agency collects through its public website and how the agency uses and protects that information. It applies to information a visitor chooses to send through the website, such as a contact-form enquiry. Care-related information is handled separately from the public website. This notice is about website data practices, not the agency's complete privacy practices for client records.

2. Information We Collect on the Website

The website contact form asks only for information a visitor chooses to provide: Name; Email address; Phone number; A general enquiry message. The website does not ask visitors to provide medical or health details. Visitors do not create accounts, and the public website does not store contact-form submissions on the site itself. A submission is routed to the agency so staff can respond.

3. What NOT to Send Through the Website

Please do not include medical or health information in a website form. Do not send diagnoses, medications, medical conditions, or personal care details through the website. Care conversations happen privately by phone at (984) 231-1623. Keeping those conversations off the public website helps protect a family's privacy and gives the agency a better way to understand what support may be needed. Important: The website should not be used for medical emergencies. If there is an emergency, call 911.

4. How We Use Website Information

The agency uses website contact information only to: respond to the visitor's enquiry; schedule a free consultation; follow up about the agency's services. The agency does not sell or share website contact details for marketing or other unrelated purposes.

5. How Care Information Is Handled Separately

Once a family becomes a client, care-related information is kept in the agency's secure records system. Access is limited to authorized staff who need the information for their work. Client care information is not stored on or transmitted through the public website. Employees' clinical check-offs are documented by the supervising nurse and retained in personnel files. These records are maintained separately from the public website and its contact forms.

6. Employee Information

The caregiver application collects employment information only. It does not ask applicants to submit health information through the public website. After an application is received, employment paperwork is requested and handled securely.

7. Retention and Access

Website enquiries are retained only as long as needed to respond and to keep appropriate business records. The agency limits access to people who need the information to respond or perform their work. Client care records are retained according to the agency's record retention policy. That policy is separate from the handling of website enquiries.

8. Your Choices

A visitor may ask the agency to correct or delete the details from a website enquiry. Please call (984) 231-1623 or email seedinghopehha@gmail.com. The agency may need to confirm enough information to identify the enquiry before making a change.

9. Updates to This Notice

The owner/administrator reviews and updates this notice. The effective date is shown below. If the agency changes how it handles website information, the updated notice will reflect those changes.

10. Related Documents and Where to Find Them

This Privacy Notice is published on the agency's website together with the following documents, each of which is linked in the website footer and should be read alongside this notice: Terms of Use — the rules for using the website. Cookies Policy — the cookies and similar technologies used on the website, and how to control them. Accessibility Statement — how the agency is working to make the website usable for everyone, and how to ask for help. If any part of this Privacy Notice appears to conflict with the Terms of Use on a matter involving personal information, this Privacy Notice controls for that subject.

Note on third-party services: the website does not currently include a live chat feature. If one is added, the agency will display a visible notice telling visitors not to enter health or medical information into it. Website forms are designed not to collect medical or health information. Do not enter health details into any website feature.
